How it works:
You can purchase a book from FOMT for $20.00 each. On each page of the book is a different location. Take the book into as many locations as possible by August 22nd21st. At each location you go to, tell the employee that you have a book and you need to draw a chip. You will pull one poker chip out of the bag, and the employee will write down the color of the chip, the date, and their initials on their page in the book. On August 21st you will need to turn in your book, at the 5th Annual F.O.M.T. Poker Run, before 4pm. The point value of each chip will be determined at the Poker Run.
Just by turning in your book, you are automatically entered into a lottery to win up to $100.00
